Social Prescribing

Your doctor isn’t the only person who can help you feel better. You can improve your own health and well-being through social prescribing.  

You don’t need a formal referral from your GP either; you can contact the service direct where you will be given the opportunity to meet a community connector who can spend time with you exploring what activities and local support could improve your health and well-being. You can then consider the benefits of participating in them and we will assist you in planning your next steps.

 

Supporting a range of issue such as;

  • loneliness
  • social isolation
  • emotional wellbeing
  • healthy life choices
  • getting out and about
  • life changing events such as a birth, retirement, bereavement
  • long-term health conditions
  • loss of confidence/purpose
  • poor health linked to housing conditions
  • accessing wok, training and volunteering
